The OpenTeleHealth solution manage many conditions, including: Diabetes, Chronic Obstructive Pulmonary Disease (COPD), Hypertension, Asthma, Congestive Heart Failure (CHF) and Pregnancy.

Basically all clinical conditions can be described and monitored using the graphical design tool that is used for creating and editing protocols (questionnaires).

Using the solution for remote patient monitoring will result in reducing number and length of hospital days, stabilizing patients as well as it can reduce the risks of complication and inhibit deterioration of patient’s condition.

It is important that the protocols need to be geared toward the specific patient group in order to be effective. A generic setup will not provide the cost reductions described above.

DIABETES

Patients with congestive heart failure (CHF) run a high risk of exacerbation and repeated hospitalization. By using the software for long-term chronic disease self-management, patients’ lives can be dramatically improved.

ASTHMA

Manage asthma with the valuable Asthma Control Test and Peak Flow measurement using a peak flow meter device. After completing measurements, the patient will receive an evaluation of their current condition and how well he manages and treats his asthma. The patient will be notified if he has trouble managing his asthma and must seek help.

PREGNANCY

Collect clinical health data from pregnant women at home or in remote locations. The fetal monitoring health data is available in real time on the OpenTeleHealth web portal. Based on clinical review, necessary actions can be taken upon data that is beyond the individual patient’s threshold values.

SCREENING

Support screening of large groups. Screening interventions are designed to identify disease in a community early, thus enabling earlier intervention and management in the hope of reducing symptoms and mortality. If the healthcare professional is able to precisely describe which questions to answer and what data to collect, from a patient, then any condition can be monitored in principle. This can range from post-OP procedures monitoring, be it knee replacements or cardiac surgery. A knee replacement patient will answer questions about the knee – does it feel warm etc. whereas the cardiac patient will be met with questions about ability to exercise and similar clarifying questions.
