OpenTeleHealth to heart failure patients

The Northern Denmark Region now offers remote monitoring to patients with heart failure. The project is an inter sectorial collaboration between the North Jutland municipalities, the Region, the general practitioners and Aalborg University. The project builds on the initial project for COPD-patients, which is called TeleCare Nord.

The heart failure patient will receive a kit with a tablet, a scale and a blood pressure gauge and the nurse or the heart failure ambulatory will receive and review the patients’ health data.

The Northern Denmark Region is a frontrunner in their use of telemedicine. Because of their positive experience with using telemedicine for COPD patients, telemedicine for COPD patients is now enrolled nationally in Denmark.
